Туннельные синдромы на верхних конечностях при сахарном диабете (обзор литературы)

Туннельные синдромы являются самой частой формой поражения периферических нервов верхних конечностей при сахарном диабете и нередко протекают незаметно и вызывают стойкие моторные нарушения, ухудшая при этом течение и прогноз диабетической полиневропатии верхних конечностей. В данном обзоре продемонстрированы самые частые фокальные невропатии верхних конечностей, встречающиеся при сахарном диабете: карпальный и кубитальный синдромы.

Несмотря на то что термин «диабетическая невропатия» известен с 1864 г., первое объединенное международное определение этого заболевания было согласованно только 124 года спустя.

На международной конференции, посвященной проблеме диабетической невропатии (ДН), в 1988 г. в городе Сан-Антонио диабетическая невропатия была определена как клинические или субклинические признаки поражения периферической нервной системы у пациентов с сахарным диабетом (СД) при отсутствии других причин их развития [4].

Однако это понятие отражает только этиологическую основу ДН и не учитывает патогенетических и клинических особенностей этого заболевания.

ДН является неоднородным заболеванием и включает в себя несколько форм поражения периферической нервной системы, отличающихся между собой по механизмам патогенеза и по клиническим проявлениям патологического процесса. Существуют три основные формы поражения периферических нервов при СД — это дистальная симметричная полиневропатия (ДСП), фокальная невропатия (ФН) и проксимальная амиотрофия [34].

Эти формы могут развиться у некоторых пациентов изолировано, а у других комбинировано. При этом дифференциальная диагностика этих форм является немаловажной задачей при выборе подходящей терапии.

ДСП является самой частой формой поражения периферических нервов нижних конечностей при СД. Данная форма поражает верхние конечности крайне редко и развивается чаше всего у пациентов с тяжелым течением СД. Однако у многих пациентов с СД развиваются жалобы на слабость и онемение в руках, которые ошибочно принимаются как ДСП верхних конечностей [8]. При применении электронейромиографического исследования (ЭНМГ) периферических нервов в диагностике этих случаев у большинства пациентов выявляются патологические изменения со стороны периферических нервов верхних конечностей, характерные для ФН [1, 2, 26]. Преимущественно эти изменения находят при исследовании срединного нерва в карпальном канале (карпальный синдром) и локтевого нерва в кубитальном канале (кубитальный синдром) [15].
