Артропластика тазобедренного сустава является золотым стандартом лечения остеоартроза тазобедренного сустава [1], а также одной из самых распространенных операций во всём мире [2]. В связи со старением населения и увеличением распространенности остеоартрита [3] с каждым годом наблюдается тенденция к увеличению числа проводимых операций [4], целью которых является уменьшение болевого синдрома, увеличение функциональности и подвижности сустава, что в совокупности ведет к повышению качества жизни пациентов [5]. На важность восстановительного лечения для увеличения мышечной силы, улучшения динамических стереотипов и качества жизни пациентов в послеоперационном периоде после эндопротезирования тазобедренного сустава указывают многие авторы [6–9], а эффективность реабилитации подтверждена рядом исследований [10–13]. Таким образом, биотехническое совершенствование имплантов, большой научный и клинический опыт, накопленный за последние два десятка лет, указывает на необходимость оптимизации существующих протоколов реабилитации в послеоперационном периоде и поиска новых технологий для эффективного восстановления большого числа пациентов.
Нами был проведен анализ исследований, изучающих современную модель реабилитации после эндопротезирования за период 2019–2022 гг. Принципиально новых подходов к протоколу реабилитации и маршрутизации пациентов в послеоперационном периоде выявлено не было. Продолжает обращать на себя внимание тот факт, что ряд авторов [14, 15] оценивают важность fast-track терапии для восстановления физиологического стереотипа ходьбы [14], и данный подход к лечебной тактике не утрачивает свою актуальность, отмечая ускоренный прирост дистанции ходьбы и значительное уменьшение выраженности болевого синдрома в послеоперационном периоде [16], что в сочетании с клиническим уходом эффективно улучшает удовлетворенность пациентов хирургическим вмешательством в целом [17]. К интересному выводу по поводу предоперационной реабилитации пришли ученые Yin H, Chen B, по мнению которых, ее проведение не влияет на отдаленные результаты проведенной операции и, несмотря на небольшое уменьшение интенсивности ранних послеоперационных болей, не несет большого клинического значения [18]. С учетом необходимости в реабилитации большого числа оперированных пациентов в мировой медицинской практике, проводится активный поиск эффективных форм и технологий по послеоперационному восстановлению. Так, Cetinkaya Eren O, Buker N своим пациентам после операции выдавали обучающие видео и учебные буклеты, которые были подготовлены в соответствии с рекомендациями для пациентов после тотального эндопротезирования тазобедренного сустава (ТЭТС), при этом результаты их исследования показали, что обучение по видео после выписки и образовательные буклеты снижают уровень боли и кинезиофобии у пациентов, а также повышают функциональность и удовлетворенность пациентов [19]. В литературе нашло отражение исследование, где авторы указывают на необходимость предоставления информации пациентам по особенностям восстановления после операции, особенно указывая на необходимость персонификации подаваемой информации до выписки и более тщательного междисциплинарного наблюдения для улучшения контроля за реабилитацией в течение первых 6 недель после операции. Кроме того, авторы отмечают возможность телекоммуникации для индивидуальной обратной связи [20].
