Проблемы подготовки пациентов к колоноскопии (обзор литературы)

Неадекватная подготовка толстой кишки приводит к неоправданной потере рабочего времени и средств на проведение исследования, а также подвергает пациентов дополнительным рискам, связанным с повторной колоноскопией и потенциально возможным развитием осложнений. В настоящей статье проведен критический обзор современных данных литературы с целью разработки индивидуальных алгоритмов подготовки для пациентов из групп риска. Поиск литературы проводился в базах данных PubMed и E-library с использованием ключевых слов сolonoscopy, сolon cleaning, bowel preparation, quality of colonoscopy, «колоноскопия», «подготовка». Сделано заключение, что перспективным направлением является применение индивидуализированных схем подготовки к колоноскопии. Дополнительную роль также может сыграть обучение пациента — образовательные буклеты, видеоролики или текстовые сообщения по особенностям индивидуальной подготовки.

В настоящее время колоноскопия является методом выбора в диагностике заболеваний толстой кишки. Ее роль как инструмента скрининга доброкачественных и злокачественных новообразований невозможно переоценить [1].

Адекватная подготовка кишечника является одним из основных условий современной эндоскопической диагностики, необходимых для полной визуализации слизистой оболочки толстой кишки и выявления ее пораженных участков. Неадекватная подготовка толстой кишки приводит не только к неоправданной потере рабочего времени и средств на проведение исследования, но и подвергает пациентов дополнительным рискам, связанным с повторной колоноскопией и потенциально возможным развитием осложнений [2–5]. В исследовании Соловьева М.В. и соавт. показано, что неадекватная подготовка пациента в 6–27% случаев приводит к ложноотрицательным результатам эндоскопического исследования [6]. Кроме этого, при неудовлетворительной подготовке толстой кишки информативность эндоскопического исследования прогрессивно снижается: количество выявленных аденом сокращается на 40–50%, а у 80–90% пациентов возникает необходимость повторных исследований в течение последующего года наблюдения [7].

Для оценки качества подготовки кишечника было разработано несколько шкал, однако наиболее распространенной является Бостонская шкала, утвержденная на основании результатов многочисленных клинических исследований [8, 9]. Эта шкала позволяет оценить подготовку каждого из отделов толстой кишки от 0 до 3 баллов, где высший показатель соответствует отличной, а 0 — неудовлетворительной подготовке.

Для улучшения качества подготовки были разработаны специальные стандартные схемы приема препаратов, очищающих кишечник. Тем не менее у некоторых категорий пациентов эти алгоритмы не позволяют достичь оптимальных результатов подготовки. Отдельными авторами было показано, что возраст, пол, а также сопутствующие заболевания в анамнезе: сахарный диабет, гипертоническая болезнь, ожирение, хронические запоры, неврологические заболевания, перенесенные операции на органах брюшной полости являются значимыми факторами риска неадекватной подготовки толстой кишки [10–12]. Учитывая необходимость разработки индивидуальных алгоритмов подготовки для больных из групп риска, нами был проведен критический обзор современных данных литературы.
