Каждому врачу, независимо от его специальности, неоднократно встречались такие пациенты, у которых отмечались жалобы на патологию со стороны слизистой оболочки полости носа, а все виды проводимого обследования с поиском аллергенов оказывались отрицательными. Такая ситуация ставит практического врача, будь то аллерголог, оториноларинголог или врач общей специальности, в тупик. В какую группу отнести такого пациента? С одной стороны, раз тесты отрицательные — по логике, это состояние должно бы называться «неаллергическим ринитом»? С другой стороны, пациент четко указывает на обострение, к примеру, при контакте с кошкой, и тогда это же вроде бы аллергический (причем атопический по происхождению) ринит? Наша статья как раз и посвящена разъяснению подобной ситуации.
В настоящее время в большинстве национальных стандартов, протоколов и руководств принято определение аллергического ринита (АР) как наличие по меньшей мере двух из четырех симптомов (в виде заложенности носа, насморка, чихания и зуда) дольше одного часа в день на протяжении по крайней мере двух недель [1–5]. Эти подгруппы ринитов принято подразделять на собственно аллергический и на неаллергический риниты [6,7]. В свою очередь, неаллергические риниты (НАР) чрезвычайно гетерогенны и включают в себя длинный перечень состояний: профессиональные, вкусовые или густаторные, атрофические, возрастные (elderly-), гормональные и идиопатические [8–10]. В нашей же стране, с ее столицей — городом Астана, находящимся на втором месте по холодовому признаку в мире среди столиц различных государств (после Улан-Батора как столицы Монголии), из идиопатических ринитов преобладают холодовые вазомоторные состояния [11]. Объединяющей же характеристикой неаллергических ринитов является только локализация процесса, то есть слизистая оболочка полости носа [12,13].
