The current economic situation is such that the implementation of new, but already proven management systems guarantees increase in efficiency with minimal energy and economic costs for treatment and preventive institutions. The quality of health care services becomes much better. The most simple and effective way to achieve these goals is a quality management system based on the requirements of the international standard ISO 9001: 2015.
Internal auditing is a tool for management control of the quality of health care. Analysis of international standards and scientific literature using the system/process approach allowed internal auditing to be presented as a system, a management function and a process. The organization of the internal auditing is the basis of the quality management system for the quality of health care in a health care institution.
In case of major emergency situations, the following sequence of arrivals of the various teams is the best option for response to medical consequences: emergency response groups with special equipment and having the skills of delivering first aid to the victims right in the accident cluster; specialists of the regional centre for disaster medicine; sufficient number of emergency medical service teams. Emergency response groups, specialists from regional centres for disaster medicine, emergency medical service teams, air medical teams and in-patient facilities are personally involved in the response to medical consequences of major emergency situations. Each of these participants has specific tasks, the implementation of which is directly related to the system of preparedness, which is achieved in the course of daily activities in the field specific to each participant.
Influenza is an acute infectious respiratory illness caused by the influenza virus. It enters the group of acute respiratory infections (ARI). It occasionally spreads in the form of epidemics and pandemics. Currently, more than 2 000 variants of the influenza virus with different antigenic spectrum are identified. According to the WHO estimates, 250 to 500 thousand people die from all variants of the virus during seasonal epidemics in the world (most of them are over 65 years old). In some years the number of deaths can reach one million. This article is devoted to the issues of antiviral treatment and prevention of influenza in adults, the advantages of Ingavirin in the treatment of influenza.
The article reviews the main aspects of antithrombotic strategies in prevention of systemic embolism. The role of using the CHADS2 / CHA2 DS2 -VASc score in the process of deciding on the choice of the antithrombotic prophylaxis method is evaluated. The ratio between benefit and risk is investigated when using oral anticoagulants in patients with a low risk of embolic complications. The effectiveness of double antiplatelet therapy is described, as well as the use of new oral anticoagulants. The review discusses clinical situations in which the use of some forms of antithrombotic therapy is preferred. The effectiveness and safety of Warfarin, monitoring of INR, its target values are assessed.
Each head of a health care institution bears a huge responsibility for organizing occupational health and safety within the walls of the institution entrusted to him or her. Any health care institution is not inferior to any industrial enterprise in terms of the number of dangerous and harmful factors: it is the work of elevators and hoist lifts, autoclave rooms and nutrition departments, vehicles and high-tech equipment. One category of health personnel is exposed to ionizing radiation during the performance of their professional duties, while others experience significant static load during operation. Also it is necessary to take into account the need to use poisonous and toxic substances as reagents and consumables, drugs and disinfectants, possible contact with the causative agents of viral and bacterial infections, and serious psychological stress. The current labour legislation regulates the duties of the employer with regard to the protection of the life and health of health workers. These duties are presented in the Labour Code of the Russian Federation (Article 212), Federal Law of 30.12.2001 No. 197-FZ and Order No. 126 of the Ministry of Health of the Russian Federation of 29.04.1997 “On the organization of occupational health and safety in management bodies, institutions, organizations and enterprises of the system of the Ministry of Health of the Russian Federation”.

Management of the quality system at any enterprise, engaged in the production of goods or delivery of services, provides, among other things, the feedback from the customer. In this case, health care institution acts as a service provider, and patient is the customer of this type of service. Organization of correct and effective system of health services in such institution depends on how satisfied the patient is after visiting the health care institution. The principle of modern health care, focused specifically on the patient, is the following postulate: the patient must leave the walls of the health care institution not only healthy, but also satisfied with the quality of provided health care.
The organization of video surveillance in treatment and preventive institutions should not be a private whim of the head, but a necessary and strictly observed norm. Public health institutions are objects of increased danger from the point of view of criminal risk. This is due to the relatively free access of all visitors, the availability of expensive equipment, valuable materials and medicines that are in limited access, personal belongings and clothing of patients and health care personnel. Only in one year the murder of the patient by a doctor and the theft of a baby from an obstetric hospital by a student were committed in general hospitals in one of the cities of Russia. The presence of surveillance cameras allowed to quickly restore the course of events and identity the criminals, who received a well-deserved punishment. We will tell about requirements, that should be put forward to the video surveillance system, and documents, that regulate the organization of this process in health care institutions.

Any medical instrument that comes in contact with the human body, whether it is an invasive intervention or a simple manipulation on its surface, must be sterile. Part of the medical instruments, intended for single use, is sterilized in the factory by the manufacturer, and placed in sealed packages, which guarantee the maintenance of sterility during the specified shelf life. Another type of medical instrument is intended for sterilization immediately before use already within the walls of a health care institution. In this case the instruction for the use of these products should contain information on the method and mode of sterilization. Finally, there is a wide range of medical instruments, which undergoes repeated sterilization after pre-cleaning. The existing methods of sterilization, the issues of storing medical instrument after the sterilization cycle will be discussed in this article.
