The use of drugs during pregnancy and breastfeeding remains one of the least studied, but important issues in pharmacology today. When choosing medicinal products during pregnancy, one of the crucial principles is to ensure the safety of the embryo and fetus. Among the generally accepted rules aimed at eliminating fetal risk, a decrease in the dosage of the drug to the lower limit of the therapeutic range is usually named. However, one should take into account the changes that occur in the body of a pregnant woman: the body weight and the volume of circulating blood rise, the excretion of drugs is accelerated (the clearance increases), which, on the contrary, requires an increase in the dosages of individual drugs to maintain therapeutic efficacy. Antibacterial drugs (ABD) are the most difficult group of drugs to use, since they require taking into account not only the mechanism of action of drugs, but also the basics of clinical microbiology. ABD occupy a special place in clinical practice, as they are one of the most frequently prescribed classes of drugs.

Беременность и лактация — периоды, отмечающиеся в жизни практически каждой женщины. Возникающие в этот период различные заболевания, как специфически характерные, так и экстрагенитальные зачастую требуют медикаментозного лечения.

Применение лекарственных средств во время беременности и при кормлении грудью на сегодняшний день остается одним из наименее изученных, но важным вопросом фармакологии. При выборе ЛС во время беременности одним из важнейших принципов является соблюдение безопасности эмбриона и плода. Среди общепринятых правил, направленных на исключение фетального риска, обычно называют снижение дозировки препарата до нижней границы терапевтического коридора. Однако следует учитывать и изменения, которые происходят в организме беременной женщины: повышается масса тела, объем циркулирующей крови, ускоряется выведение (повышается клиренс) препаратов, что требует, наоборот, увеличения дозировок отдельных препаратов для сохранения терапевтической эффективности.

Антибактериальные препараты — самая трудная для применения группа препаратов, так как они требуют учета не только механизма действия препаратов, но и основ клинической микробиологии. АБП занимают особое место в клинической практике, поскольку являются одним из наиболее часто назначаемых классов ЛС. По данным ретроспективных исследований, частота назначения антимикробных препаратов беременным женщинам составляет от 20 до 30 % [1]. Выбор антибиотиков во время беременности, с одной стоны, зависит от характера возбудителя, тяжести инфекции, особенностей фармакокинетики ЛС, степени его проникновения через плаценту, с другой стороны, от степени риска возникновения повреждающего действия на плод с развитием тератогенного и эмбриотоксического действия, что существенно ограничивает перечень АБП, назначаемых при беременности.

Большинство антибактериальных препаратов имеют низкую молекулярную массу и легко проникают через плаценту, создавая терапевтические концентрации в крови плода, сопоставимые с концентрацией препарата у матери. Вопрос безопасности воздействия антибиотиков разных групп на плод исследован не окончательно. На сегодняшний день существует общепринятая классификация антимикробных препаратов по степени их безопасности для плода — классификация Американского управления по контролю за безопасностью продуктов и лекарств (FDA).
